Please can we make some change to wording on the complaint form, see current and new suggestion below.
Current questions design:
  1. Please describe in as much details as possible the clear details of your complaint ie the main issue with the medical advice/treatment/service you have received and exact details what happened including any dates, times, who was involved:
  2. Please kindly describe how this situation has affected you/others?
  3. Please describe what you feel the practice should do to help try and resolve this issue going forwards?
NEW question design:
  1. Could you please explain, as clearly and specifically as possible, what it is about the service you received that you were not satisfied with?
(This helps us understand your main concerns.)
  1. Can you describe what happened that you feel we could have handled better?
(If possible, please include any relevant dates, times, and names of staff involved.)
  1. While we don’t need detailed clinical information, is there anything else that would help us better understand your experience so we can learn and improve?
